POSITION SUMMARY
The HPM physician will work in many facets of the palliative care domain. The HPM physician will serve as the physician member of a hospice interdisciplinary team(s)(IDT) that will include visits to private homes, nursing homes, hospice facilities, hospitals, or wherever the patient resides. This role will also include providing palliative care consultative services and serve as the physician member of the IDT. As the palliative care consultant, the physician will provide direct medical care that will include pain/symptom management, leading family meetings, advance care planning, HPM education to hospital staff and community, and serving as a collaborating physician for nurse practitioners and other members of the IDT both in the inpatient and community setting. Additionally, the HPM physician will get to plan and implement new and innovative programs that serve the seriously ill.
