The Primary Care Physician will be assigned to Medical Service under the supervision of the Section Chief of Primary Care and Chief Medical Service. This position will be located at Northport VAMC. The Primary Care Physician is responsible for providing outpatient examinations, diagnosis, and therapeutic ability, effectiveness in emergencies, patient management, consultation, specialty skills and record keeping Responsibilities include, but not limited to: He/she will perform and record history and physical examinations and record data from clinic visit; evaluate, consult, develop diagnoses, and manage the primary care panel of patients assigned; enter progress notes in computerized medical record and completes workload documentation in a timely manner; provide medical care encompassing acute and chronic medical conditions and all appropriate preventive care for patients; orders diagnostic tests, medications, laboratory tests as medically relevant for each patient; provide timely follow up on results, including completion of view alerts; determine medical assessment and develop treatment plans according to accepted protocols; prescribe medication according to approved formulary, and in accordance with Federal and State law; may collaborate with an assigned mid-level provider and complete collaborative case reviews as assigned; assess veterans by utilizing advanced clinical judgment and physical assessment skills; identify acute/non-acute/chronic health problems. Lab, ECG, PFT, cardiac stress tests and X-ray follow-ups Technical Aspects This physician will perform history and physical examination, evaluate problems and define the appropriate therapeutic approach. This physician will see outpatients in the outpatient clinic and at the Riverhead CBOC. This physician is responsible for support, cooperation and participation in meeting the goals and objectives of the Medical Care Cost Recovery Program. This includes assuring the cooperation of subordinates in performing tasks which may directly or indirectly contribute to the accomplishment of the MCCR collection goals and the fulfillment of the requirements of Public Laws, VA Directives, legislation and local initiatives which govern the MCCR program. The incumbent will ensure that a safe, efficient, and high quality health care is delivered within the department. He/She will ensure appropriate initial evaluation, treatment, and consultation as well as documentation, consistent with current VHA directives and medical center policies. The Incumbent will demonstrate sound decision making skills, both clinical and administrative, which bring credit to the profession and serve to enhance the distinctive and contributory role of physicians. Demonstrates effectiveness in teaching, monitoring and coordination of educational activities, i.e., planning evaluating and documenting, i.e., a. Attending Physician b. Preceptor for Medical Students and Residents. Administrative Responsibilities The incumbent will be responsible for other vital roles including: communication with patients and their families, patient education, discharge planning and communication with outpatient providers, arranging follow up, preparing discharge instructions, medication reconciliation, and ensuring communication during hands off of care. He/ She will maintain appropriate clinical privileges in order to fulfill the duties of assignment within Medical Service. May attend on the Inpatient Service and perform in-patient Medical consult if the need arises; Participates effectively in: Program planning, administrative judgment, decision willingness, correspondence and/or reporting.
