Plans, implements, and evaluates nursing care, utilizing nursing process for assigned patients. Provides for psychosocial needs of patient.
Â Essential JobÂ Duties:
Performs nursing assessments.
Performs individual, family, and group psychoeducation.
Develops nursing related programs as appropriate.
Performs initial precertifications and concurrent reviews on PHP patients.
Tracks daily attendance, admissions and discharges of PHP patients.
Reviews, updates and participates in the development of treatment plans.
Interacts with community agencies to establish mental health network.
Assists the Assessment and UR department as needed and able (e.g. answering RESPOND calls).
Follows the chain of command and serves as the patient advocate within applicable laws and patient rights.
Reviews charts for physician orders and assures orders are completed as necessary.
Implements the individual interventions from the plan of care.
Communicates and coordinates services with supervisor.
